Astigmatism explained simply: why everything looks slightly stretched

Almost one adult in three is astigmatic to some degree. Most have no idea what the word refers to, and many wrongly assume it is something serious.

An eye that sees sharply works like a perfectly round ball: the light entering it converges on a single point on the retina. In an astigmatic eye, the cornea, the transparent surface at the front of the eye, is not perfectly spherical. It curves a little more in one direction than the other, like a rugby ball lying flat rather than a football.

The consequence: light does not converge on one point, but along two slightly offset lines. The resulting image is never entirely sharp, at any distance.

How it shows up

The typical symptom is not outright blur, but an image that is slightly stretched or doubled. Car headlights sprout rays at night. Letters look alike, an 8 can be mistaken for a 0, an H for an N. Text seems to bleed a little.

The second symptom, more common still, is indirect: fatigue. Uncorrected mild astigmatism forces the eye to adjust constantly to compensate for the blur. That invisible work produces headaches at the end of the day, a feeling of tension around the eyes, and a drop in concentration when reading.

This is why many people with mild astigmatism simply think they are tired. They see "more or less fine", so they never get checked.

Reading astigmatism on a prescription

A prescription carries three values per eye. The sphere, written SPH, corresponds to short-sightedness if it is negative and long-sightedness if it is positive. The cylinder, written CYL, gives the amount of astigmatism. The axis, written AXIS and expressed in degrees from 0 to 180, gives the direction of the distortion.

A cylinder of 0.25 is negligible. From 0.75 upward, correction clearly changes comfort. Above 2.00, the astigmatism is marked and the correction calls for precise centring.

The axis is what sets astigmatism apart from other corrections. A lens for astigmatism is not symmetrical: it has to be mounted at an exact orientation. An error of a few degrees at glazing, or a frame that has twisted and now sits crooked, is enough to degrade vision. It is an important thing to check when you collect a pair.

How it is corrected

Toric lenses have a surface calculated to compensate exactly for the difference in corneal curvature, along the required axis. Technically they demand more manufacturing precision than a simple lens, which explains a slight extra cost.

For contact lenses, toric versions exist, designed to stay stable on the eye and not rotate. They are thicker at one specific point, which makes them reposition themselves with every blink. These are the lenses you need, as a standard spherical lens does not correct astigmatism.

Refractive surgery can also treat astigmatism in some cases. That is a medical decision, taken after a full assessment, and outside the scope of this article.

Three misconceptions

Astigmatism gets worse with screen use. False. It comes from the shape of the cornea, which changes slowly with age and not with use. Uncorrected astigmatism is, however, more noticeable when you are tired.

You can be astigmatic without being short-sighted. True, and it is common. The three corrections are independent: you can have astigmatism on its own, or combined with short-sightedness, or with long-sightedness.

Astigmatism corrects itself in children. False, and this is the most important point. Significant uncorrected astigmatism in the early years can stop the eye learning to see sharply, which is called amblyopia. Early screening is essential.

The adaptation period

A first astigmatism correction often feels strange: the floor seems curved, vertical lines look tilted, and your sense of distance is disturbed for a few days. That is normal, the brain has to unlearn the compensations it has been making for years.

This phase usually lasts from two days to two weeks. If the discomfort continues beyond that, the axis or the power deserves a check. Do not settle for glasses you cannot wear.
